If you are an established patient, please message us through  our portal at  elationpassport.com, click patient log in in the upper right corner of the site and sign in with your email and password.   Files containing medical records, identification/insurance cards or other needed documents can be sent by starting a new message and clicking the paper clip in the lower left hand corner where copies  of these can be attached.
